“President Obama’s Long Form Birth Certificate

In 2008, in response to media inquiries, the President’s campaign requested his birth certificate from the state of Hawaii. The state sent the campaign the President’s birth certificate, the same legal documentation provided to all Hawaiians as proof of birth in state, and the campaign immediately posted it on the internet. That birth certificate can be seen here (PDF).
When any citizen born in Hawaii requests their birth certificate, they receive exactly what the President received. In fact, the document posted on the campaign website is what Hawaiians use to get a driver’s license from the state and the document recognized by the Federal Government and the courts for all legal purposes. That’s because it is the birth certificate. This is not and should not be an open question.
The President believed the distraction over his birth certificate wasn’t good for the country. It may have been good politics and good TV, but it was bad for the American people and distracting from the many challenges we face as a country. Therefore, the President directed his counsel to review the legal authority for seeking access to the long form certificate and to request on that basis that the Hawaii State Department of Health make an exception to release a copy of his long form birth certificate. They granted that exception in part because of the tremendous volume of requests they had been getting. President Barack Obama’s long form birth certificate can be seen here (PDF):

Donald Trump continues to speak out about Obama’s birth certificate, eligibility issues and connections to Tony Rezko. CNN, NPR and the media in general continue to lie and cover for Obama. The Rod Blagojevich second trial begins this week on April 20. Donald Trump, if you are watching, here is a repint from Citizen Wells on March 14, 2011.
This is the first article in a series about corruption in the US Justice Department and specifically an operation I refer to as “Protecting Obama.” Many events smelled back in 2008, but with the passage of time, the stench is corroborated. Below is the Evidentiary Proffer, a summary and timeline of the bulk of evidence that reveals how Obama was protected in the investigation and prosecution of Tony Rezko, Rod Blagojevich, et al. The time is right to present my case. There are enough well documented events to make a strong case. As I have mentioned recently, I have contacted a prominent congressman about this issue and others and this is a way to get the facts in front of the congressman, his staff and the public. The Proffer will certainly be updated from time to time. I will next present an opening statement and then go on to present the case in more detail. However, anyone who has been paying careful attention or who reads this Proffer, will come to the obvious conclusion presented here of collusion involving the US Justice Department, Patrick Fitzgerald, Barack Obama, and probably Rod Blagojevich and Tony Rezko.
Before reading the Proffer, here is a recap of how important Rezko is to the prosecution of Blagojevich and ultimately indicting Obama.
It is understandable why the Blagojevich defense team does not want Rezko on the stand. Judge Zagel’s comment about Rezko being a bad witness reeks of conspiracy. Convicted felons of all categories are regularly used as witnesses. Stuart Levine, the key witness in the Rezko trial was not only enmeshed in corruption but was a long time drug user.
Here are the approximate number of times that “Rezko” is mentioned in US Justice Department documents.
Even if a strong argument is made against using Rezko as a witness, and good luck with that argument, Stuart Levine was used extensively as a witness in the Rezko trial, and his name was mentioned approx. 146 times in the Blagojevich Criminal Complaint. And remember, Blagojevich’s name was mentioned at least 30 times on one day of the Rezko trial.

Auchi, now a citizen of the United Kingdom, has faced criminal charges in Europe. He also figured in the revocation of Rezko’s bond early this year after attempting to wire him more than $3 million. Upon learning of that attempt, U.S. District Judge Amy St. Eve declared Rezko a flight risk and ordered him held in a federal jail in the Loop.
The Rezko party in 2004 was designed to induce Auchi to pour money into the South Loop investment. Obama’s presence at the party was not previously known. “FBI Special Agent Daniel Cain, the primary case agent on the investigation into Levine and Rezko, is on the stand now in testimony that is laying the foundation for entering the wiretaps into evidence.
Cain said the investigation, dubbed Operation Board Games by the federal agents, began in December 2003 and was prompted by information gleaned from an informant whom he did not identify. That witness, he said, took part in meetings with two other individuals who were in contact with Levine by phone at his home.
Cain said Levine had three phone lines in his North Shore home. Federal agents recorded conversations on those lines April 8-May 21, 2004. Those dates span the time when Levine, Rezko and others allegedly were working to rig the hospital board vote on a Mercy Health System hospital proposal for Crystal Lake and other kickback schemes prosecutors claim they were engaged in.” April 21, 2004: (Rezko Trial March 11, 2008; 5:27 p.m.)
“The former lawyer for a state board that regulates hospital expansions took the stand late in the afternoon and said she told Stuart Levine of her concern after an odd vote on the Mercy Hospital project that included Levine whispering to other board members before the project was approved.
His response was no less troubling, she said.
“He shrugged his shoulders and said, ‘Sometimes you have to be a good soldier,’” said Anne Murphy, the former lawyer for the Illinois Health Facilities Planning Board.
Murphy said Levine’s remark followed a vote that saw the plan for a new hospital in Crystal Lake squeak by with the minimum of five votes in favor on the nine-member board.
With eight members present at the April 2004 meeting, Murphy said the vote was stalled with three members voting “yes,” two voting “no” and two abstentions when Chairman Thomas Beck was called to give his vote.
Beck said, “Where’re we at?” Murphy testified. He then went over to whisper with Levine off the record, she said. When that conversation ended, Murphy said, Levine got up and whispered to Imad Almanaseer.
Beck then voted yes, Murphy testified, and Almanaseer changed his vote to “yes,” giving the plan the necessary votes to pass.
“There was an audible, collective gasp across the room,” Murphy recalled of the April 2004 meeting.
Prosecutors contend the five members who voted for the hospital were a Levine-led bloc that Rezko had placed on the board and controlled. The two were charged with arranging to accept a kickback from the builder who was going to build the Mercy project in Crystal Lake.
The entire board was new as of the summer of 2003, Murphy said. She said she had given the new members memos on ethics, including conflicts of interest.”
“Another government wiretap has been played with Antoin “Tony” Rezko’s voice on it, and this one could prove damaging to his defense. On the tape, recorded May 18, 2004, Rezko can be heard giving orders to political fixer Stuart Levine about how he wanted to manipulate the vote of one of Levine’s fellow members on the Illinois Health Facilities Planning board, Danalynn Rice.
June 7, 2004: Stuart Levine, member of the Illinois Health Facilities Planning Board, abruptly resigns. This is the first indication that the Blagojevich administration is under federal criminal investigation.
July 2, 2004: Stuart Levine resigns from the state Teachers’ Retirement System board. July 27, 2004: Obama Democratic Convention Speech. May 9, 2005: Stuart Levine indicted on corruption charges. Federal subpoena issued to Tony Rezko. June 15, 2005: Obama purchases home next door to Rezko for $1.65 million, $300,000 less than the asking price. June 15, 2005: Rita Rezko, Tony’s wife purchased plot next door for $625,000 asking price. Sept. 15, 2005: Joseph Cari, former lawyer for the state’s teacher’s pension board pleads guilty to extortion. A high ranking public official is described in court documents only as “Public official A.” Oct. 25, 2005: The Chicago Tribune reports about a federal grand jury investigation into the alleged political hiring practices of the Blagojevich administration. Late 2005 early 2006: From the Kenneth J. Conner complaint against Mutual Bank filed Oct. 16, 2008. The lawsuit is still active.
“11. In late 2005 or early 2006, Conner performed an appraisal review of the Adams Appraisal (Exhibit C) per the directive of Richard Barth and James Murphy. Conner prepared a written Appraisal Review report (ARR) opining that the Adams Appraisal overvalued the Greenwood lot by a minimum of $ 125,000.00″
“18. On October 23, 2007, eight days after Conner’s October 15, 2007 email to Schlabach attached as Exhibit J, Mutual Bank terminated Conner’s employment for pretextual reasons.”
Conner later told World Net daily when he initially was fired, that the bank and the Rezkos were engaged in “fraud, bribes or kickbacks, use whatever term you want,” to benefit the Obamas.
Soon after the Blagojevich arrest, Conner was interviewed by investigators from Fitzgerald’s office. Jan. 2006: Rita Rezko sells the Obamas one-sixth of her lot for $104,500. Feb. 4, 2006: (Chicago Sun-Times)
“Illinois Gov. Rod Blagojevich has always been rumored to be interested in higher office. A single entry in a campaign-finance report he filed this week could add fuel to the speculation.
Blagojevich paid $7,500 in December to one of the top Democratic political strategists in Iowa, where contacts are important for anyone with presidential aspirations.” Feb. 19, 2006: Blagojevich began his 2006 re-election to IL Governor. Aug. 5, 2006: The Chicago Tribune reports that Stuart Levine is cooperating with the federal investigation of state government. June 29, 2010: Judge Zagel said there was a word to describe witnesses like Rezko who damage whatever side calls them to testify.

Aug. 17, 2010: Blagojevich convicted of 1 of 24 counts. Neither Rezko or Levine were called as witnesses. One juror held out from indicting on more counts. The trial was much shorter than predicted . Approx 2% of the wiretaps were presented. Jan. 6, 2011: Rezko sentencing delayed indefinitely. Jan. 28, 2011: Rezko sentencing set for October 21, 2011. Feb. 24, 2011: Counts 1,2,4 in the Blagojevich Indictment are dropped. This represents approx. half of the indictment and includes numerous corruption ties to Blagojevich and Obama.
The only reference left to “Hospital” in context of Mercy Hospital was in count one. March 9, 2011: Blagojevich files motion to be sentenced on one conviction charge.

A cursory reading of this evidentiary proffer reveals a completed puzzle picture, a large billboard that reads:
Barack Obama was enmeshed in the Chicago corruption of Rod Blagojevich and Tony Rezko and was a participant in the rigging of the IL Health Planning Facilities Board. This well planned board rigging, which took place over many months, was a large part of the indictment and trial of Tony Rezko and the early legal charges against Rod Blagojevich.
The facts and reference to this rigging have diminished over time in the Justice Department legal documents and prosecution pleadings.
The prosecution of Rod Blagojevich has been repeatedly delayed.

(Lafayette Hill, PA – 04/13/11) – Philip J. Berg, Esquire, the first Attorney who filed suit against Barack H. Obama on August 21, 2008 challenging Obama’s lack of “Constitutionally Eligibility” to serve as President of the United States stated that Obama’s sister, Maya, was treated lightly last night, April 12th on CNN’s Piers Morgan Tonight show as she was not asked why on two [2] different interviews she stated that her brother, Barack or rather Barry, was born in two [2] different hospitals in Hawaii.
Berg said, “Apparently, Piers Morgan, has joined the so called ‘Free Press’ and media by avoiding the hard facts regarding where Obama was born. It is a shame that Piers Morgan did not live up to his reputation of being a tough interviewer.
Obama’s sister, Maya Soetoro-NG was interviewed by Bennett Guira of Rainbow Edition, Volume 2, Issue 3 in November 2004. In this article, entitled “A New Face in Politics”, it is stated in the first paragraph “Barack Obama was born on August 4, 1961 at the Queen’s Medical Center in Honolulu, Hawaii.” [emphasis added] However, Maya Soetoro-Ng was interviewed again by Laurie Au with the Start Bulletin in February 2008 under the headline “Obama backers stress importance of caucuses”. In the Star Bulletin interview, Maya Soetoro-Ng stated:
“This is a place that’s been relatively unchanged for him, and so much of who he is was born here,” Soetoro-Ng told a crowd of about 200 people eating “OBAMAsadas.”
“He was born in Kapiolani Medical Center for Women & Children, two blocks up from where our grandmother still resides … the place where he used to stick orange peels under his bed, where it got all dried out and … nasty.” [emphasis added]
Berg said, “Piers asked soft questions of Maya regarding where her brother was born and completely failed to address and/or question the contradicting statements by Obama’s sister.”
Berg continued, “I am so proud of Donald Trump as he has raised the issue of Obama’s birth even hoping that Obama will prove he was born in Hawaii. However, Trump has stated that the more he looks into the issue, the more he believes that Obama was not born in Hawaii.
Therefore, a perfect time Tuesday night for Piers to ask the tough question that he failed to do. I am sure that many people tuned in to Piers last night to see Piers ask Maya the question that has haunted her since 2008. Piers failed !”
Berg said, “I believe Piers did not want to be involved in the “Birther” issue, but many people tuned in to see the rare interview of Maya. Piers gave Maya and Obama a break they did not deserve.

“She found the original birth record, properly numbered, half typed and half handwritten, and signed by the doctor who delivered Obama, located in the files.”
Why does Fukino tell us that the real birth certificate showed the doctor on it but she fails to tell us that Obama’s birth occurred in a hospital which is what Obama told the public occurred? After all, she knows how much “birthers” have been demanding to see conclusive proof that Obama was born in Kapi’olani Hospital, the hospital in which Obama says he was born. There would be no violation of any privacy laws for her to simply say that he was born in a hospital, without giving the name of the hospital, just like she said that a doctor delivered Obama and signed the birth certificate. What is suspect is that there were numerous doctors at that time who could have delivered Obama but only two hospitals in which he could have been born, Kapi’olani Hospital or Queens Hospital. In other words, the doctors get lost in the shuffle but the hospitals do not. Just saying that a doctor signed the birth certificate does not open up channels of information that can be independently verified. On the other hand, knowing the birth hospital pins down the place of birth to only two specific locations in which there would be contained medical evidence of the alleged birth there. …”

Thank God there are intelligent people like Donald Trump who can see through the Orwellian lies being spun about Obama and his birth certificate deficiency and other eligibility issues. Recently Michael Isikoff reported an alleged interview of former Hawaii Health Dept. Director Dr. Chiyome Fukino, who herself has either been misquoted or has been lying or extremely confused. Note in the article there is no mention of the reporter name or date of the interview. Here are a few Orwellian snippets.
“The Hawaiian state health official who personally reviewed Barack Obama’s original birth certificate has affirmed again that the document is “real” and denounced “conspiracy theorists” in the so-called “birther” movement for continuing to spread bogus claims about the issue.”
My Response: There may be a real birth certificate on file. We are anxiously awaiting a certified copy for confirmation of location of birth.

But Wisch, the spokesman for the attorney general’s office, said state law does not in fact permit the release of “vital records,” including an original “record of live birth” — even to the individual whose birth it records.

My Response: This is loaded with lies & Orwellian misinformation.
A COLB can be obtained for a non Hawaii birth. There has been no official verification that the COLB place on the internet was obtained from Hawaii.
Here is the latest big Orwellian lie being spread. “state law does not in fact permit the release of “vital records,” including an original “record of live birth” — even to the individual whose birth it records.”
Common sense dictates that the actual birth certificate filed with the state cannot be released. A certified copy with details will suffice. This will resonate with most people who, like myself, were required to produce a certified copy of my original birth certificate for activities like Little League Baseball.
“Moreover, on Oct. 13, 2010, a contributor with the username “Danae” posted on FreeRepublic.com a copy of her long-form birth certificate originally issued in 1969, but which the Hawaii Department of Health mailed to her on Sept. 28, 2010, after she paid a $10 fee.”
“To verify her transaction, she also posted a receipt she was sent Sept. 28, 2010, by the Hawaii state agency, documenting the $10 fee to get her long-form, hospital-generated certificate.”
“Because of the obvious questions raised by the statements of Hawaii officials defending the secrecy of Obama’s birth records, “Danae” followed up with a photograph of her original long-form, hospital-generated certificate of live birth side-by-side with the copy she was sent.”
“She went even further, posting an image of both of those documents along with a short-form COLB, similar to the one presented by Obama’s campaign, that she also had asked the Hawaii Department of Health to produce:”
“”Danae” obtained her long-form, hospital-generated certificate of birth to win a $200 bet with another FreeRepublic participant operating under the username “STARWISE.” STARWISE dared someone to prove he or she could get a long-form birth certificate from the Hawaii DOH after 2001, when the Hawaii DOH went electronic and agency spokeswoman Janice Okubo claimed they quit issuing such documents.
In fact, the Hawaii Revised Statutes, Paragraph 38-13(a) specifies that the agency “shall, upon request, furnish an applicant a certified copy of any certificate, or the contents of any certificate, or any part thereof.” Further, subparagraph C allows that copies of birth certificates “may be made by photograph, dry copy reproduction, typing, computer printout or other process approved by the director of health.”
Further, according to a report in “The Post & Email” blog by Miki Booth, others have gotten long-form documents from Hawaii, too.
“When my son, Alan, requested a copy of his birth certificate he got just that – a “Certificate of Live Birth,” not the same as Obama’s. Different titles and different information. Obama’s is sorely lacking information required for obtaining a passport – Mother’s State/Country of Birth and Father’s State/Country of Birth,” she wrote.
Further, she wrote about the experience of a friend, whose name was not revealed, just this year. She released an image of a “Certificate of Live Birth” that appears to be a long-form document with much additional detail not included in a short-form “Certification of Live Birth.”
She said the long-form “Certificate of Live Birth” was obtained from Hawaii in March 2011.”
What about these statements from Neil Abercrombie, the Governor of Hawaii, and Tim Adams, an election official in Honolulu in 2008.

He has since signed an affidavit swearing to his allegations.”
“The radio hosts suggested that Janice Okubo, the public information officer for the state health department had previously vouched for the authenticity of the COLB, though in reality she has refused to do so. But Adams maintained the online scanned image is fraudulent.

Trump has been right in bringing up Obama’s eligibility/natural born citizen issue, the ‘birther’ issue. No one else (at this point in time) has the clout or the guts to tackle it. No other politician or ethical reporter will cover this issue in any depth because the Political Correctness (PC) police have been out in full force intimidating all who might dare to raise any questions. They’ve been very successful too. That’s why ‘birther’ is the name everyone associates with it; and why most associate the meaning the name implies – not born in Hawaii. No independent thought necessary! However, it is an issue that needs to be addressed, if for no other reason that we have military men and women fighting around the world at Obama’s beck and call.

Unfortunately, there is no Supreme Court decision that I know of where the Court addresses the specific question (in a case where it is factually necessary to the facts and issue presented) of who can and can’t be a Natural Born Citizen, other than in the penumbra of discussions/historical presentations in cases involving ordinary and/or naturalized citizenship.
However, the relevant writings/common understandings that were available to the Founders and the ones that have been suggested that the Founders relied, suggest (as stated by Prof. Solum in ‘Originalism and the Natural Born Citizen Clause’) that to be a natural born citizen requires being born in country to parents (2) who were citizens at the time of the person’s birth.”

Form DS-10: Birth Affidavit
If you were born in the United States and cannot present primary evidence of U.S. citizenship, you may submit Form DS-10: Birth Affidavit as evidence of your U.S. citizenship. The birth affidavit:
Must be notarized
Must be submitted in person with Form DS-11
Must be submitted together with early public records
Must be completed by an affiant who has personal knowledge of birth in the U.S.
Must state briefly how the affiant’s knowledge was acquired
Should be completed by an older blood relative
NOTE: If no older blood relative is available, it may be completed by the attending physician or any other person who has personal knowledge of your birth
If you claim citizenship through birth abroad to U.S. citizen parent(s), but cannot submit a Consular Report of Birth Abroad or Certification of Birth, you must submit all of the following:

The arrest of IL Governor Rod Blagojevich on December 9, 2008

common denominator among all of these corruption figures is
Barack Obama. Obama has many long time close ties to Tony Rezko
and conspired with on or more of Rezko, Levine, Weinsein and
Blagojevich to rig the IL Health Planning Facilities Board. The
indictments or in the case of Blagojevich, criminal charges,
reveal the common link. The first article of this series examines
in detail the corruption of this board and serves as a request
to Patrick Fitzgerald to indict Barack Obama. Mr. Fitzgerald’s
office was contacted and the article was faxed to him.
Subsequent articles will reveal new details in the ongoing

Donald Trump was interviewed on the Sean Hannity Radio Show yesterday, April 7, 2011. Trump continued to speak out about the Obama eligibility and birth certificate issues. Trump also brought up the land deal between Obama and Rezko.
