“Ebola: Liberia deaths ‘far higher than reported’ as officials downplay epidemic”

“The true death toll from the Ebola epidemic is being masked by chaotic data collection and people’s reluctance to admit that their loved ones had the virus, according to one of west Africa’s most celebrated film-makers.

Sorious Samura, who has just returned from making a documentary on the crisis in Liberia, said it is very clear on the ground that the true number of dead is far higher than the official figures being reported by the World Health Organisation.

Liberia accounts for more than half of all the official Ebola deaths, with a total of 2,458. Overall, the number of dead across Liberia, Sierra Leone and Guinea has exceeded 4,500.

Samura, a television journalist originally from Sierra Leone, said the Liberian authorities appeared to be deliberately downplaying the true number of cases, for fear of increasing alarm in the west African country.

“People are dying in greater numbers than we know, according to MSF [Médecins sans Frontières] and WHO officials. Certain departments are refusing to give them the figures – because the lower it is, the more peace of mind they can give people. The truth is that it is still not under control.””

“Samura believes sexual promiscuity among westerners could play a role in the virus’s spread abroad. Almost immediately after the outbreak was reported in March, Liberia’s health minister warned people to stop having sex because the virus was spread via bodily fluids as well as kissing.“I saw westerners in nightclubs, on beaches, guys picking up prostitutes,” he said. “Westerners who ought to know better are going to nightclubs and partying and dancing. It beggars belief.It’s scary.”

He said another striking feature was that the ineffectiveness of years of aid had been laid bare: “Money has poured in from the west, but it has gone to waste. Ebola should make us think about how the west gives aid to Africa; aid has not been used to create a system able to cope with this challenge. Ebola has exposed the fact it is not working. That money has gone to waste.””

“U.S. Ebola Cases May Exceed Two Dozen by November, Researchers Say”

“There could be as many as two dozen people in the U.S. infected with Ebola by the end of the month, according to researchers tracking the virus with a computer model.

The actual number probably will be far smaller and limited to a couple of airline passengers who enter the country already infected without showing symptoms, and the health workers who care for them, said Alessandro Vespignani, a Northeastern University professor who runs computer simulations of infectious disease outbreaks. The two newly infected nurses in Dallas don’t change the numbers because they were identified quickly and it’s unlikely they infected other people, he said.

The projections only run through October because it’s too difficult to model what will occur if the pace of the outbreak changes in WestAfrica, where more than 8,900 people have been infected and 4,500 have died, he said. If the outbreak isn’t contained, the numbers may rise significantly.”

““We have a worst-case scenario, and you don’t even want to know,” Vespignani said. “We could have widespread epidemics in other countries, maybe the Far East. That would be like a bad science fiction movie.”

The worst case would occur if Ebola acquires pandemic status and is no longer contained in West Africa, he said. It would be a catastrophic event, one Vespignani said he is confident won’t happen.”

“A second health care worker at Texas Health Presbyterian Hospital who cared for Thomas Eric Duncan has tested positive for Ebola, health officials said Wednesday — casting further doubt on the hospital’s ability to handle Ebola and protect employees.

The worker reported a fever Tuesday and was immediately isolated, health department spokeswoman Carrie Williams said.

The preliminary Ebola test was done late Tuesday at the state public health laboratory in Austin, and the results came back around midnight. A second test will be conducted by the Centers for Disease Control and Prevention in Atlanta.

“Health officials have interviewed the latest patient to quickly identify any contacts or potential exposures, and those people will be monitored,” the health department said.

But the pool of contacts could be small, since Ebola can only be transmitted when an infected person shows symptoms. Less than a day passed between the onset of the worker’s symptoms and isolation at the hospital.

Official: Duncan should have been moved

An official close to the situation says that in hindsight, Duncan should have been transferred immediately to either Emory University Hospital in Atlanta or Nebraska Medical Center in Omaha.

Those hospitals are among only four in the country that have biocontainment units and have been preparing for years to treat a highly infectious disease like Ebola.

“If we knew then what we know now about this hospital’s ability to safely care for these patients, then we would have transferred him to Emory or Nebraska,” the official told CNN Senior Medical Correspondent Elizabeth Cohen.

“I think there are hospitals that are more than ready, but I think there are some that are not.””

“Texas Health Worker Tests Positive for Ebola”

“A health care worker here who helped treat the Liberian man who died last week of the Ebola virus has tested positive for the disease in a preliminary test, state health officials said Sunday.

“We knew a second case could be a reality, and we’ve been preparing for this possibility,” said Dr. David Lakey, the Texas health commissioner. “We are broadening our team in Dallas and working with extreme diligence to prevent further spread.”

The worker, who was not identified, was an employee of Texas Health Presbyterian Hospital in Dallas, where the first person diagnosed with Ebola in the United States, Thomas E. Duncan, died last week.

The health care worker reported a low grade fever Friday night and was isolated and referred for testing. Officials interviewed the worker and were identifying “any contacts or potential exposures,” the statement read.”

“Ten of the 48 people were considered high risk, including seven health care workers, Mr. Duncan’s fiancee, Louise Troh, and two other relatives and community members, all of whom were confirmed to have contact with Mr. Duncan. The other 38 are considered low risk, and include people who may or may not have had any direct or indirect contact. The low-risk group included health-care workers who drew or processed Mr. Duncan’s blood at the hospital as well as a homeless man who rode in the ambulance that took Mr. Duncan to the hospital after the vehicle dropped him off but before it was taken out of service and disinfected.”

“4 Things We’ve Learned About Enterovirus D68, And 1 Mystery”

“On Aug. 15, doctors and nurses at Children’s Mercy Hospital in Kansas City, Mo., realized they had a problem.

Children were coming into the emergency room with an illness that caused wheezing and breathing problems so severe that some children ended up in the ICU on ventilators. And it was spreading fast.

“By Aug. 20, we were beyond our bed capacity,” Dr. Mary Anne Jackson, chief of the infectious diseases section at Children’s Mercy, said at the IDWeek meeting in Philadelphia.

Since then, the virus that caused those illnesses, enterovirus D68, has sickened children across the country and may be responsible for the deaths of five children. Jackson and Dr. Aaron Milstone of Johns Hopkins Children’s Centers briefed reporters on the outbreak Thursday morning.

Bottom line: There are still a lot more questions than answers about the outbreak and how the virus might affect children’s health in the future, but we’re learning. Here’s what infectious disease specialists have found out to date.

This is the largest outbreak of EV-D68 ever recorded. Cases have been reported in 45 states and the District of Columbia. Though the CDC has confirmed 664 cases as of Wednesday, there are undoubtedly many, many more cases that are causing milder coldlike illnesses. “This is different this year, what we’re seeing,” Milstone said. “Something is different.”

So far it’s impossible to know what’s making it different, Milstone says — if the virus has changed so it’s more dangerous or if it’s spreading so much more widely that we’re seeing more cases, both mild and severe.”

Dr. Austin King, head of the Texas Medical Association, is now calling out Texas Governor Rick Perry and President Barrack Obama to allow doctors to screen all of the unaccompanied children who have been coming across the border from Honduras and El Salvador. The President has remained silent with regard to the request.

Dr. King expressed the view that in order for more doctors to go down to the border, emergency funding would likely need to be approved, and ICE would have to change some policies in order to allow everyone coming across to be screened.

Dr. Elizabeth Lee Vliet, a prominent preventive medicine physician, who previously served as the former Director of the Association of American Physicians and Surgeons, stated that “Legal immigrants are required to pay for medical exams and screening tests conducted by approved physicians before they arrive here. They’re not arriving in the U.S. carrying diseases, and then being dispersed into cities across the nation.” The CDC acknowledges this very fact on their website. Vliet further stated that the CDC’s own guidelines are clearly being violated with regards to the illegal immigrants currently crossing the border.”

“About 20″ cases of a polio-like syndrome have been identified in California children over the past 18 months, a Stanford University researcher says.
Dr. Keith Van Haren, a pediatric neurologist at Lucile Packard Children’s Hospital Stanford, has written a report about five of the early cases.”

“However, some types of enterovirus are more serious. These can cause hand, foot and mouth disease; viral meningitis; encephalitis (inflammation of the brain); an infection of the heart; and paralysis in some patients.”

Human rhinoviruses (HRVs) belong to the Picornaviridae family with high similarity to human enteroviruses (HEVs). Limited data is available from Latin America regarding the clinical presentation and strains of these viruses in respiratory disease.

Methods

We collected nasopharyngeal swabs at clinics located in eight Latin American countries from 3,375 subjects aged 25 years or younger who presented with influenza-like illness.

Results

Our subjects had a median age of 3 years and a 1.2:1.0 male:female ratio. HRV was identified in 16% and HEV was identified in 3%. HRVs accounted for a higher frequency of isolates in those of younger age, in particular children < 1 years old. HRV-C accounted for 38% of all HRVs detected. Phylogenetic analysis revealed a high proportion of recombinant strains between HRV-A/HRV-C and between HEV-A/HEV-B. In addition, both EV-D68 and EV-A71 were identified.

Conclusions

In Latin America as in other regions, HRVs and HEVs account for a substantial proportion of respiratory viruses identified in young people with ILI, a finding that provides additional support for the development of pharmaceuticals and vaccines targeting these pathogens.

“Ebola is ‘entrenched and accelerating’ in West Africa”

“The World Health Organization (WHO) has warned that Ebola is now entrenched in the capital cities of all three worst-affected countries and is accelerating in almost all settings.

WHO deputy head Bruce Aylward warned that the world’s response was not keeping up with the disease in Guinea, Liberia and Sierra Leone.

The three countries have appealed for more aid to help fight the disease.

The outbreak has killed more than 3,860 people, mainly in West Africa.

More than 200 health workers are among the victims.

Speaking on Thursday, Mr Aylward said the situation was worse than it was 12 days ago.

“The disease is entrenched in the capitals, 70% of the people affected are definitely dying from this disease, and it is accelerating in almost all of the settings,” he said.”

“The Republican Party is taking Sen. Kay Hagan to court to find out if she used her influence for a company owned by her husband to get special treatment from the federal government. A company owned by Kay Hagan’s husband, Chip, and his brothers received $390,000 in stimulus money.

Maybe it’s time that somebody looked into the fact that Chip Hagan was also a party to a lawsuit against the Piedmont Triad Regional Water Authority, which provides water to Greensboro, High Point, Randleman, Randolph County, Archdale and Jamestown.

Chip Hagan’s group sued, and the judge who made the original ruling in the case at that time had been nominated by Kay Hagan for a lifetime appointment as a federal judge. The ruling went against the water authority and, in a settlement, the water authority agreed to pay Chip Hagan and the owners of other downstream hydroelectric dams $2.35 million.

Kay Hagan later dropped her support for part-time Superior Court Judge Calvin Murphy’s appointment, but three weeks before the ruling was made Hagan had nominated him. So when Murphy made the ruling in favor of Chip Hagan’s company and the other owners of small dams downstream who were represented by Hagan’s law firm, Murphy was recommended by Kay Hagan for the prestigious and lucrative lifetime appointment as a federal judge.

It was only after it became public that Kay Hagan had nominated a judge who made a controversial ruling in favor of her husband’s company that Kay Hagan withdrew the nomination.

The eventual settlement of the lawsuit cost the water users of Greensboro $1.25 million.

The executive director of the Piedmont Triad Regional Water Authority at the time of the ruling, John Kime, was quoted in newspaper reports as saying, “It was clear to me that we weren’t getting an unbiased opinion, based on politics.”

One of Kay Hagan’s husband’s companies gets $390,000 in federal funds and another gets an extremely favorable ruling by a judge who she had nominated to the federal bench, but Kay Hagan knows nothing about either.”

“At least five children infected with the respiratory illness known enterovirus D68 (EV-D68) have died in the U.S. in the past month.”

“The lastest confirmed victim was a four-year-old New Jersey boy, Eli Waller. He died at home on September 25. The Centers for Disease Control (CDC) confirmed the cause of death Friday night. But health officials say they have no idea how he contracted the virus. A health official says Eli was “asymptomatic and fine” when he went to bed but died overnight. He had no known preexisting immune weakness.”

“The CDC reports that in the past month and a half, at least 538 people in 43 states and the District of Columbia have become ill with EV-D68. Most of them are children and some developed limb paralysis. Polio, which can cause paralysis and death, is a type of enterovirus. EV-D68 is one of more than a hundred “non-polio” enteroviruses.”

Several weeks ago, National Nurses United began surveying registered nurses across the U.S. about emergency preparedness.  Most of the nurses are telling NNU that their hospital is not prepared for the Ebola virus.”

“Ebola in Spain: 4 people including nurse hospitalized in Madrid”

“Health officials in Madrid say three more people are in the hospital on suspicion of contracting Ebola. The news comes a day after a nurse who treated two Ebola patients at a city hospital became infected with the disease.

The nurse is now being treated with a drip using antibodies from those previously infected with the virus, Reuters reports. Approximately 22 people who have been in contact with the woman, dubbed by media the “Spanish Ebola nurse,” have been identified and are being monitored, Madrid health officials told reporters Tuesday.

The officials added that the hospitalized include the nurse’s husband, another health worker and a traveler who had spent time in one of the affected West African countries.”

“Spanish authorities have come under increasing pressure to explain how the disease was able to spread in one of their hospitals. While they say all proper procedures were followed while providing care to the deceased missionaries, reports to the contrary have surfaced.

According to The Guardian, staff at the hospital said waste from the rooms of both patients had been carried out in the same elevator used by all personnel. The hospital was also reportedly not evacuated when the second patient, García Viejo, was taken in to receive treatment.

Union workers also accused the government of providing hospital staff with inadequate protective clothing.

Some Spanish health workers’ representatives said the situation should prompt an overhaul of procedures and facilities used to treat those afflicted with the virus.

“Something went wrong,” Máximo Gonzalez Jurado, head of Spain’s General Nursing Council, told Spanish news agency EFE. “They need to establish if the protocol is correct or not correct so that a case like this, that never should have happened, doesn’t happen again.””

It is too bad we are spending so much time and resources fighting ISIS, a problem that Obama, et al, allowed to exacerbate.

It is too bad these so called religious fanatics, who if they truly had religion would be assisting to help fight Ebola.

It is too bad that Muslims don’t do a better job of policing those who use Islam for an evil agenda.

It is too bad that poor people in Africa must suffer more.

We must protect our country and rally the world to fight Ebola, in an intelligent, compassionate manner.

“Devastating news from the Ebola clinic”

“Today we are filming at the country’s main referral hospital – Connaught Hospital in central Freetown. As we enter, I see a woman in a purple and pink shirt lying on a bench, with her head in her hands. She looks extremely unwell. This area is where patients showing symptoms of Ebola come for help, but the help is limited.

This isn’t a treatment centre; it’s an isolation ward within the hospital. People have to travel many miles from here by ambulance to get proper supportive treatment. There are just 18 beds in this hospital, and they are all full.

The latest patient to arrive is a one-month-old baby. Ebola killed both his parents overnight. The chances are he is also infected and will die within days. All medics can do is feed him and hold him through protective suits. I am reminded of my trip to Guinea a couple of months back, when I was covering this outbreak. Back then, I watched the body of a four-month-old baby lowered into the ground. Ebola also killed his mother. It’s heart-breaking to imagine the most likely outcome for this other tiny baby.”
“As we are leaving the hospital, a black truck pulls up. The burial team is here to remove two bodies and bury them in the nearby cemetery. We watch and then follow the makeshift hearse to these victims’ final resting place.

A whole area is cordoned off just for suspected and confirmed Ebola victims. Walking into it is eerie and tragic. There are hundreds of graves, most dug very recently, with fresh mounds of mud on top of them. One or two have a cross or children’s toys scattered on them. Most, though, are unmarked. What hits me is the sheer scale – 400 bodies buried here in a matter of weeks.

The burial team is efficient and almost jovial. I imagine it’s the only way they can keep performing this grim task day in, day out. The cemetery supervisor, Abdul Rahman Parker, tells me he’s been ostracised by his community – people are scared of him now because he handles the bodies of Ebola victims. But he says he doesn’t care, and that Sierra Leone needs him to continue doing this job, even if its people don’t realise it.”
“My brother Francis is sick, and they won’t take him at this centre. They say they are full. What are we supposed to do? We’ve been travelling from hospital to hospital all day and no-one will take him.”

I peer into the car. Francis is sitting in the passenger seat staring into space. His eyes are red, and he has the hiccups – both are clear symptoms of Ebola. After almost an hour of pleading, the family eventually give up. The five of them pile back into their car and drive away. Everyone in that vehicle is now potentially at risk of catching Ebola.

When we enter the treatment centre, I feel the helplessness and frustration of that family and I demand to know why they didn’t allow that potentially dying man inside. Surely they can do something for him. The centre’s co-ordinator, Luca Rolla, tells me their priority has to be their staff and the patients they are already treating. He tells me that they cannot go over capacity or they risk everyone else inside the centre. One of their doctors has already contracted the virus and is now being treated in Germany.

It’s an impossible choice for these medics, and my frustration quickly pales in comparison to theirs. Luca has taken the family’s details and if a bed becomes free anywhere in or around Freetown, he will let them know.

Luca tells me, what’s needed right now is more international medics and training of local medics, and more isolation centres. Until then – he says – he will have to continue turning patients away, knowing full well they risk going back into the community and infecting yet more people.”
“Then soon after 18:00, just as one of the BBC World presenters is about to introduce me live, my producer, Mark, runs over and tells me some terrible news. Francis Samuka, whom we watched being turned away from a treatment centre yesterday, has died. His family has called and told us he passed away at an isolation centre a few hours ago. His sister could barely speak when she was delivering the news, she was wailing with sorrow. My heart sinks… and then I hear the presenter in my earpiece saying: “Tulip, what’s the latest?”

I explain what’s happened, all the time thinking of Francis’ bloodshot eyes and the look of despair I saw in him just the day before.

I am glad we were able to tell Francis Samuka’s story. It’s important people know this is happening on a daily basis across West Africa. It underlines why governments here and global aid agencies continue to plead for more international help, so patients like Francis can be treated, instead of being turned away.”

Following reports that a Dallas hospital failed to hospitalize a patient infected with the Ebola virus and failed to properly communicate essential information to caregivers about his health status, National Nurses United is stepping up the call on U.S. hospitals to immediately upgrade emergency preparations for Ebola in the U.S.

Several weeks ago, National Nurses United began surveying registered nurses across the U.S. about emergency preparedness.  Most of the nurses are telling NNU that their hospital is not prepared for the Ebola virus.

In updated preliminary results from nearly 700 RNs at over 250 hospitals in 31 states released Friday:

  • 80 percent say their hospital has not communicated to them any policy regarding potential admission of patients infected by Ebola
  • 87 percent say their hospital has not provided education on Ebola with the ability for the nurses to interact and ask questions
  • One-third say their hospital has insufficient supplies of eye protection (face shields or side shields with goggles) and fluid resistant/impermeable gowns
  • Nearly 40 percent say their hospital does not have plans to equip isolation rooms with plastic covered mattresses and pillows and discard all linens after use, less than 10 percent said they were aware their hospital does have such a plan in place
  • More than 60 percent say their hospital fails to reduce the number of patients they must care for to accommodate caring for an “isolation” patient

NNU is calling for all U.S. hospitals to immediately implement a full emergency preparedness plan for Ebola, or other disease outbreaks. That includes:

  • Full training of hospital personnel along with proper protocols and training materials for responding to outbreaks,
  • Adequate supplies of Hazmat suits and other personal protective equipment
  • Properly equipped isolation rooms to assure patient, visitor and staff safety,
  • Sufficient staffing to supplement nurses and other health workers who need to care for patients in isolation.

At a rally of 1,000 nurses last week in Las Vegas, “we warned that it was just a matter of time in an interconnected world that we would see Ebola in the U.S. Now, everyone should recognize that Texas is not an island either, and as we’ve heard from nurses across the U.S., hospitals here are not ready to confront this deadly disease,” said NNU executive director RoseAnn DeMoro.”
